Monthly Direct Primary Care Membership Prices
A membership with 702 DPC and Wellness covers all standard office visits with no co-pays. This applies to individuals, families, and for patients registered by their employers as part of our employee health benefit plan.
Enrollment Fee At Start of Membership
$100 per person. $150 for couples and families.

* Payment of monthly membership fee is required to remain an active member and receive service
* THERE ARE NO LONG TERM CONTRACTS OR COMMITMENTS. A 30 day notice for cancellation is all we ask.
* Enrollment fee will need to be repaid to become a member again after 6 months of cancellation date

Most importantly, you will have a provider that values an open and honest relationship with their patients. All costs for membership, labs, meds, imaging (x-rays, CT scans, and MRI’s) will always be fully disclosed to the best of our ability.
* Please note that in-office procedures such as suturing, warts and age spots treatment, joint injections, skin tag removals and muscle/joint injections may be performed for an extra fee.
